Our office provides the latest cutting edge programs for anti-aging, aesthetics, nutritional counseling, weight management and smoking cessation.               

Payments

Fees are payable at time services are rendered unless other arrangements have been made in advance. We accept personal checks: name, address and phone number must appear on all checks. Returned checks are subject to a $35.00 processing charge. We also accept Visa, MasterCard and Discover cards.

Cancellations

When making an appointment with our office you will be required to provide us with a valid credit card number to secure your appointment time. In the event that cancellation is necessary we require 24 hour notice in order to allow scheduling in other patients during that time. In the event that our office is not notified within 24 hours of the cancellation, there is a $50.00 charge.  If you have a scheduled consultation for any Aesthetic procedure and you do not notify our office within 24 hours of an appointment cancellation you will be charged a $75.00 fee.
